Full job description

The Nursing Supervisor is responsible for overseeing and educating subordinate staff, completing patient assessments, start of care/intake paperwork, etc. as well as acting as a Clinical Liaison for patients and their families. Maintain the quality of patient care, in accordance with the Agency’s objectives and policies, through planning, coordination, and implementation and evaluation of the Home Health services provided.

RESPONSIBILITIES

  • Coordinate, and schedule, the daily workload of Home Health Aides, utilizing Agency personnel to their fullest capacity.
  • Provide daily supervision of Home Health Aides, to ensure the quality of services and maintenance of standards.
  • Review, the clinical content, daily documentation, diagnoses, medical history, medications, and other procedures items, from the records of patients.
  • Identify, and evaluate, patient needs and appropriateness of home care, by reviewing the initial intake screening form before assignment of direct care staff.
  • Assist with case/client openings as needed.
  • See patients in the home setting, for the supervision of HHA, care plan reviews, and care updates.
  • Participate in audits, interdisciplinary team conferences and represent Agency, in the community, as requested.
  • Assist with interviewing and onboarding with staff.
  • Appraise staff performance, during their probationary period, and annually, with the Clinical Manager; provide the teaching and counseling necessary to enhance the delivery of services, and the professional development, of home health aide staff.
  • Manage and investigate any incidents including hospital transfers, falls, etc. that may arise. Report your findings to the Clinical Manager.
  • Manage and investigate any complaints, concerns, or grievances received from clients, family members, facility representatives, or staff. Report findings to the Clinical Manager.
  • Educate, train, and coach staff on company policy and proper patient care procedures upon hire and as needed. Offer performance feedback, including areas of opportunity or those needing improvement. Follow up with new hires within one week of orientation to confirm competency in the field. Communicate all concerns to Clinical Manager and/or Administrator in a timely manner.
